The eligibility form on this site asks for your name, your phone number, the county you live in, your relationship to the person who needs care, whether that person has Medicaid, and an optional note about the help you need. You choose what to put in it. We do not ask for a Social Security number, a date of birth, a medical record or a payment detail anywhere on this website.
We use what you send to contact you about care, to talk through whether you or your family member may qualify for a program, and to answer your question. We do not sell your information, and we do not share it with anyone for their own marketing.
Form submissions are delivered to us by a service provider that operates the form and emails it to our office. That provider handles the information on our behalf and is not permitted to use it for anything else. If helping you means passing details to a program administrator or a county agency, we ask you first.
The form uses a security check from Cloudflare to tell real visitors apart from automated spam. That check may set a cookie in your browser and is necessary for the form to work.
We also use Google Analytics to understand how people find and use this site, such as which pages get read and roughly which part of Michigan a visit comes from. It sets a cookie and reports to Google. We use it to improve the site. We do not use it to identify you personally and we do not sell this information. What you type into the eligibility form is sent to our team directly and is not passed to Google Analytics. These tags are loaded through Google Tag Manager, which we may also use to measure how our Google ads perform, such as whether a visit that began with an ad led to a form submission. You can opt out of Google Analytics on every site you visit using Google's browser add-on attools.google.com/dlpage/gaoptout.
You can ask us what you sent, ask us to correct it, or ask us to delete it. Call313-949-0377or emailinfo@honeybeehhc.comand tell us what you want done. You are never required to use the form. You can simply call us instead.
This website is meant for adults arranging care. It is not directed at children, and we do not knowingly collect information from them.
If this policy changes, the date at the top of the page changes with it.
